Add a comment field for constituency codes to allow for more defined infomation

When cleaning a code table which has a lot of entries in it, it would be really helpful if it didn't jump back up to the top of the table once you had cleaned one entry.

Add all fields to query list for inclusion, not just the few available today. Definitely need all addresses, adressees, phone numbers and emails to do any kind of useful list. Also need consituent ids and attributes.
