Prospect rating Note not specifically connected to its Rating in query

In Query: I am unable to output the Note for just the Rating I am querying on. In Criteria I select one Rating, but in Output I can only choose ALL Notes. Export does not allow for this, either. Can the Note for each Rating be tied more more specifically to it's Rating Category and Description?

    Agree. I only want one specific rating note to output in my query. In the current format, I get them all which then creates multiple lines in my query which are not helpful.
