Add amendment notes to amendment import

The 'Amendment Notes' field does not exist on the Amendment Import. Please can it be made available so we can import notes when importing amendments.

This is obvious functionality that should be added. And really if this is to be done properly, amendments should have attributes. All record types should be extensible by attributes.

This is an essential functionality, without which we are unable to properly calculate important revenue channels.
