In the migration stage from a hosted Raiser's Edge database to the cloud (NXT), the "Description" field (title) of documents saved in the Media tab in the Raiser's Edge database should transfer along with the document to Attachments in NXT instead of a generic "Media_1127.pdf" name it saves as the Attachment's name. This seems like a no brainer. Since this does not occur with the migration, Raiser's Edge users will have to manually edit the name from a generic "Media_1127.pdf" name to the document's correct name in NXT.
