Add date and source fields for email addessses and phone numbers

With more emails and cell numbers being added to records, we need to be able to see when an e-mail address or phone number was changed and the source of the information

    I would like to have an info source same as the address to know where the data came from.  Currently, I'm updating comments but I would like to use the same table as address to keep the info source consistent and allow querying.
